I just clicked on the video below, and saw this. Like with my other error images, this was not edited at all.

I wonder what killed NC first in it - the laughter or having the top of his head erased by a video glitch.
By now, I might have found the trick to creating such errors: refresh a YouTube video, and duplicate the tab while it's refreshing.
